Providing person-centred care
Our home nursing support is available to adults with a life limiting palliative diagnosis approaching the last 12 months of life. We provide person-centred care based on individual need and our services are available 365 days a year to accept referrals, carry out initial assessments and review assessments. We also have a team of Family and Bereavement Support Workers to carry out visits if required.
What our home nursing can offer
Our home nursing team includes a team of highly skilled Senior Registered Nurses, Registered General Nurses, Healthcare Assistants and a Patient Care and Support Co-ordinator. The team provides palliative and end of life one-to-one support overnight, or for an agreed number of hours during the day. As well as end of life care to patients with a life limiting palliative diagnosis, we provide respite care which allows families and carers to have a break knowing their loved ones are cared for and not alone.
Where is the care provided?
Our care is provided in the patient’s home, residential and nursing homes, and community and acute hospitals. Our aim is to support patients in their preferred place of care.

Self-referral
When a patient is referred to home nursing there is an initial assessment undertaken which will identify appropriate support options. What is the cost?
We do not charge patients for receiving our services however, we do rely on donations to ensure our services can continue. If you are able to make a donation, it will help us to provide care for others.
