A high standard of essential care

The aim of the support at home service is to facilitate a person’s discharge from hospital and prevent hospital admissions where possible. Our support at home services are available to adults with a life limiting palliative diagnosis approaching the last three months of life. We provide a high standard of essential care and emotional support for people with a life-limiting condition. Our aim is to find the ‘person in the patient’ and allow them to lead on their own holistic care, and encourage them to maintain a level of independence in their own home.

What can we offer?

Our support at home service can provide:

  • Up to three visits every day from qualified staff to provide palliative care in the patient’s home

  • An hour each visit to ensure quality time is spent focusing on the patient's needs and daily activities

  • Regular re-assessments by a Senior Registered Nurse

  • Referrals into our other support services including home nursing, lymphoedema and family and bereavement services.

The service we received from Hospice at Home West Cumbria staff was excellent at all times. I am very grateful for the emotional and practical support received on the night my husband died. It was a great comfort and helped make the passage of death peaceful and bearable for all of us.

Who can access the support at home service?

The service is available to people who are eligible for ‘fast track’ discharge which is supported by Continuing Health Care Funding. Referrals into the service can be made by health care professionals involved in the patient’s care, for example a GP, District Nurse, Clinical Nurse Specialist, or hospital discharge team.
