Home nursing and support at home
Our home nursing service is available for adults with a lifelimiting palliative diagnosis approaching the last 12 months of life, whilst our support at home service is the last three months of life. We provide person-centred care based on individual need and our services are available 365 days a year to accept referrals, carry out initial assessments and review assessments. Referrals to our home nursing team can be made through:

When an individual is referred to the home nursing team there is an initial assessment undertaken which will identify appropriate support options.
The home nursing team work alongside other health and social care professionals to ensure a fully integrated approach.

Support at home referral
Our support at home service is available to people who have a palliative diagnosis and are eligible for ‘fast track’ discharge which is supported by Continuing Health Care Funding.
Referrals into the support at home service can be made by health care professionals involved in the patient’s care, for example:

Hospital Discharge Team.
The patient should be eligible for Continuing Health Care Funding in order to receive the Support at Home care package.

Family and bereavement services
Hospice at Home West Cumbria’s family and bereavement service provides a range of services to patients, carers, and their families. These services are available both before and after bereavement. Our team offers emotional support to help with coping, loss, grief, and bereavement. Highly qualified staff support patients, carers, and families by providing, one-to-one emotional support sessions and group support sessions. Complementary therapies
Complementary therapies are holistic treatments which take into account a person's physical, emotional, psychological and spiritual wellbeing.
The therapies we provide to patients, carers, and the bereaved include Reiki, reflexology, therapeutic massage, aromasticks, Emotional Freedom Technique (EFT tapping), breathing, relaxation and creative visualisation. The range of therapies offered is individual to the person according to their needs.

Lymphoedema
Hospice at Home West Cumbria has a long established specialist lymphoedema service which provides assessments, delivers appropriate treatments and supports and advises palliative and non-palliative patients, their families and carers. We are the only providers of specialist adult lymphoedema services in West Cumbria.
Comprehensive assessments are carried out by our team of specialist nurses and tailored treatment packages are offered according to the needs of the patient to promote self-management. To learn more about this service, please use the button below.

Single Point of Access Team
The Single Point of Access Team are responsible for managing all incoming calls and referrals for home nursing, support at home, lymphoedema, family and bereavement services and complementary therapies.
The clinical admin team are at the centre of hospice services. They manage referrals for all our services and are also the single point of access for the North Cumbria palliative care network.
This includes managing referrals for the Community Specialist Palliative Care Team, Loweswater Suite, West Cumberland Hospital and the Cumberland Infirmary.
