Hospice at Home West Cumbria offers nursing care and support to adults with cancer and other life limiting illnesses. When faced with these situations, many people want to remain close to their families or stay in a familiar environment so our home nurses care for people in the patient’s own home, a residential care home, a nursing home or community hospital setting.
When a patient is referred to Hospice at Home West Cumbria there is an initial assessment undertaken by either a Senior Hospice Nurse or a Clinical Nurse Specialist in Palliative Care. The care provided will reflect the needs of the patient and their family or carers. We provide respite care as well as end of life care. Depending on the patient’s needs our nurses may stay overnight or for an agreed number of hours during the day. In some cases, we provide 24 hour patient care.
The home nursing team work alongside other health professionals to ensure a fully joined up approach. Our team of dedicated nurses work tirelessly to deliver the best possible care and provide access to the most appropriate support and advice.
