Hospice Services

Over the years Hospice at Home West Cumbria has become affectionately known as the “Hospice without Walls” It continues to provide services traditionally found in a hospice building in people’s homes and their local community.  This includes:

Referrals

Referrals to our services are usually through a GP, District Nurse, palliative care specialist, clinical nurse specialist or occupational therapist working within the NHS.  It is possible to self-refer however, we will always liaise with your GP of hospital consultant.

All referrals are considered and discussed with members of the multi-disciplinary team that meets on a weekly basis.

Assessment

Once a patient has been accepted, a senior clinical staff member will create an appropriate and personalised plan of care and support. Once accepted, each patient and carer will be assessed and a personalised appropriate plan of care and support will be devised by one of our senior clinical staff.

Integrated Care

Key to the success of Hospice at Home West Cumbria is its integration with the local specialist palliative care team.

Palliative care doctors, clinical nurse specialists and occupational therapists are all employed and funded by the local NHS but join with Hospice at Home staff to work as one fully joined up team. There are six clinical nurse specialists in palliative care working in West Cumbria; four in the community and two in West Cumberland Hospital. They are experts in pain and symptom control and offer support and advice to seriously/terminally ill patients, their families and professionals involved in their care. They also help and support newly diagnosed cancer patients.

A team of palliative care doctors, led by a Consultant in Palliative Medicine are also part of the team. They provide advice and support to GPs and hospital doctors and visit the patients and families with more complex needs.

In addition there is a specialist occupational therapist and an occupational therapy assistant who provide vital, timely response to patients who are struggling with aspects of daily living.
